The ETD Access Approval Form should be turned into your Graduate School ETD Administrator either in person or by email at the time you upload your dissertation/thesis.

Note: The ETD ID number (at the upper right corner of the form) is the five digit account number assigned when you create an account in the ProQuest ETD Administrator.
Surveys
All Graduate Students: In order to receive your tickets for the University Commencement Ceremony, you must complete the Graduation Student Survey.
Doctoral Candidates Only: In addition to the Graduation Student Survey above, you are required to complete the Survey of Earned Doctorates.
